Keep Your Coverage. Find Your Way Forward.

Navigating Medi-Cal changes and health challenges shouldn't be done alone. Our bilingual Community Health Workers provide hands-on guidance to secure your benefits and coordinate your care. Looking ahead to 2027, our South County Renewal Living center will offer a peaceful, supportive home for adults to recover. We are here to support your journey toward stability.

Community • Respect • Lasting Stability • Futures Renewed • Communities Restored

Rooted in South County. Built for Renewal.

We are built around people, not paperwork. Our Community Health Workers meet you where you are and stay with you, so your coverage, your doctors and your next steps stay on track. When someone has nowhere safe to recover, our South County Renewal Living recuperative care home welcomes them into a real home in their own community. Your doctors, health plan, county behavioral health and community partners keep doing what they do best; we make sure you never have to connect the pieces on your own.

Our Framework

Health • Recovery • Stability • Renewal. We unify regional resources, life skills, and local support systems to establish firm foundations for personal recovery and lasting wellness.

Uniting Local Neighbors

South County already has real strengths: St. Louise Regional Hospital, county health centers in Gilroy and Morgan Hill, a Reentry Resource Center, and Santa Clara Family Health Plan's South County Community Resource Center in Gilroy. What is missing is someone who stays with each person through the paperwork and the transitions, and a homelike place to recover nearby. CRLF is being built to fill those two gaps, not to replace anyone.

Our Method — four steps toward renewal

We provide clear, compassionate guidance through coverage changes, care transitions and recovery close to home. A bilingual Community Health Worker stays with each member, while residents of our future home receive a safe, homelike place to heal.

01
Connect & Cover

We meet members where they are, confirm their Medi-Cal, and fix what is at risk: a renewal due, a missing document, a plan or provider change.

02
Keep Care on Track

Health navigation, appointments, transportation, health education and a warm link to primary care and behavioral health.

03
Recover Close to Home

When someone has no safe place to heal, our recuperative care home offers a homelike bed in the community, with a step-down into transitional housing in the same bed. This step applies only to residents of the home.

04
Sustain & Renew

Housing navigation, benefits, life skills and follow-up, so coverage, care and housing all hold.

Focused on South County

Gilroy counted 959 people experiencing homelessness in the January 2025 Point-in-Time Count, roughly 16 per 1,000 residents against about 6 per 1,000 countywide. South County has one acute care hospital, the 93-bed St. Louise Regional Hospital, while the county's medical respite beds are in San José. Our work begins in Gilroy, Morgan Hill, San Martin and the surrounding communities, where recovering close to home has been hardest.

CRLF changes this reality by building a robust local network alongside neighbors and organizations. We ensure residents stay connected to their families and roots while they rebuild their lives, ensuring that stability and renewal remain accessible right here for everyone who calls South County home.
